All material is guaranteed to be as specified. All work to be completed in a workmanlike manner according to standard practices. Owner to carry fire, tornado, and other necessary insurance. Owner is responsible for any and all permits. Because concrete and building conditions vary and improper building practices may not be visible, along with structural defects such as leaking walls, floors, ceilings, basements, voids in the concrete, rock pockets, improper thickness, improper consolidation, and improper curing of the concrete placement, Concrete Experts, and its employees cannot be held responsible for incidental damages not caused by improper techniques on the part of Concrete Experts service personnel. Notwithstanding the foregoing, customer hereby authorizes Concrete Experts to do any additional work and provide any materials which, in the sole discretion of Concrete Experts, are essential to repair damaged areas or ensure the structural integrity that is necessary for Concrete Experts to properly perform its services and work as set forth above. Customer clearly understands that voids or rock pockets may be present in the concrete placement and that in order to stop the leak, it will be necessary to fill these voids. This may take additional material above and beyond that stated above. This information may not be known until after Concrete Experts starts the job. Concrete Experts is authorized to fill any and all voids it deems necessary to stop the water leaks. Customer further understands that water may move to the next weakest point and start a new leak. These new leaks will have to be addressed and sealed. This will be a new charge above and beyond the quoted bid. Concrete Experts is authorized to do the work necessary and provide the materials to replace and/or repair, Page 1 of 2 including but not limited to the following: framing members with dry rot, decay, or insect damage; failing concrete or masonry: neutralize rust in steel rebar: removal of imbedded wood in concrete slabs or walls: failing deck sheathing or deck diaphragm: replacing nontreated framing in direct contact with a concrete slab on grade. Customer authorizes Concrete Experts to bring that portion of the repair up to current code requirements. These additional repairs are billed at a rate of $95 per hour per man, material is billed at 20% over cost. This includes mobilization, travel time and the time it takes to gather material and dump waste material at a State approved dump site. Customer agrees to hold Concrete Experts harmless for any damage which may occur during construction. Customer further agrees to indemnify, defend and hold harmless Concrete Experts from all claims, demands, damages, liability, and costs attendant to defending against the same brought on account of any injuries or damage received or sustained by any persons or property, arising out of the gross negligence of Concrete Experts. In the event this matter is referred for collection, customer agrees to pay all costs including lost wages, court, arbitration, and attorney fees associated with collection activity incurred by Concrete Experts. Payment cannot be withheld for artistic indifference. Customer understands that services performed by Concrete Experts may entitle Concrete Experts to a mechanic’s and material men's lien pursuant to RCW Ch. 60.04, or as may be subsequently amended, upon Customer’s real property upon which labors are performed. Customer hereby acknowledges receipt of this notice pursuant to RCW 60.04.

LIMITED LIFETIME WARRANTY: Warranty is void in the event of any earthquake, ground movement, fire, or any other act of God. The concrete basement slab must be mechanically tied to the foundation wall as approved by Concrete Experts. Furthermore, the warranty is void if payment is not made in full within 20 days of work completion.
